It could be caused by :-
1. Server busy (due to heavy traffic)
2. Server is restarting
Server restarts every day at around 8:00 AM and 8:00 PM (Server time)
3. Bug
Please include :-
1. Where were you when this happen
2. What were you trying to do
3. Login name/UserID
Just in case it is a bug
